This self-score test acknowledges that Covid-19 can have severe and even fatal consequences for a range of people. Please complete this form for personal records and later reference if you need support from your relevant Union. The scores are in no way a clinical assessment, though categories are based on peer-reviewed medical research, and generate a code to communicate your campus needs to someone without disclosing specific, confidential information. Scoring close to ‘0’ helps to keep infection rates and their consequences low, though there is no judgement for any score. If you have problems completing this survey or find it inaccessible, please contact a Union rep to talk you through it.
You are under no obligation to disclose anything about yourself that you do not wish to, but it is advisable if you are an employee, to register any concerns about a return to campus with your employer, for your own records. You are protected by at least two pieces of legislation in raising any concerns or objections. If you are using this test to support a student, identify the College policies that support mutual decisions, and those where there are gaps or issues.
Because Covid-19 is highly contagious and the highest transmissions occur in households or residential facilities, risk factors may not just relate to you, but those you live with or care for, those who care for you, and the communities you live in and travel through. This is why we designed this test.
This is a GUCU self-assessment tool intended to aid your decision-making process and any discussions with line managers/colleagues about attending campus during the pandemic. While no assessment is perfect, the GUCU form is designed to take into account as wide a range of variables as possible, including many that are omitted from the College’s Covid-age assessment.
